Next Gen Liquid Biopsy: Comprehensive Analysis from a Single Tube of Blood

Liquid biopsy's potential as a non-invasive, real-time window into tumor biology remains largely untapped; this AACR Liquid Biopsy Special Conference 2024 poster shows that a single 7.5 mL blood draw can be analyzed on RareCyte's Orion spatial-biology and CyteFinder rare-cell platforms to characterize circulating tumor cells, immune-profile white blood cells, and read targeted mutations from both the tumor cells and cell-free DNA.

  • Every analyte comes from one 7.5 mL draw of blood. Whole blood is collected in an AccuCyte tube, and from that single sample three classes of analyte (circulating tumor cells, white blood cells, and cell-free DNA) are recovered and measured; this is the array of tests the poster applies to cancer samples to demonstrate clinical feasibility.
  • An 18-plex immuno-oncology panel profiles immune cells, and a core detection panel identifies the tumor cells. On the Orion spatial-biology platform the 18-plex panel phenotypes white blood cells into T-cell, macrophage and B-cell subsets; circulating tumor cells are separately characterized as nucleated, CK/EpCAM-positive, CD45-negative events, with two open channels held for tumor biomarkers such as ARv7 and PSMA in a prostate example.
  • Individual CTCs and cell-free DNA are sequenced and their mutations compared. Single cells picked from the slides with the CytePicker, together with plasma cfDNA, undergo targeted sequencing that maps driver mutations across genes including PTEN, ALK and NF1; one PTEN variant registered a 23.70% variant frequency in the CTCs and 39.10% in the cfDNA, while several other mutations were detected in the CTCs alone.
